Work Permit Information
From 6/23 - 8/22
Please go to the ABC District Website link which will direct them to the statement of intent and google form to upload their completed document:
After 8/22 (normal school year procedure)
Once you have the job, email [email protected] with the following information:
Name of employer (business name)
Address of where you’ll be working
Name of supervisor (first AND last name)
Telephone number of business
We will create the “Intent to Employ” form and email it back to you for signatures (employer, parent, student). DO NOT provide your social security number on the form.
Once we receive the completed form, we’ll create the Work Permit.
Please note that this process can take a few days, especially if the employer is not already in our database.
We will need at least 24 hours to process your request.
